We will need you to enter your insurance information AND upload a photo of the front and back of your insurance card. This will allow us to contact the insurance company and verify benefits. We will need a card on file that you (the patient) is able to upload on their own end in order to cover any copay, deductible or private payments.
Our patient portal is secure and HIPAA compliant. We will need all this information in order to upload you into the system. As a disclaimer once your registration is accepted and you go on to request an appointment your time you prefer may not be available and will need to be scheduled to a different date or time.
Once everything is complete in your patient portal you will be in contact with someone from our team via phone or email.
